Access to your own records

Under data protection legislation, you, the 'data subject', can make a subject access request to view your own patient or personnel information. An application form will be sent out to you for completion before we can start to search any records - this acts as proof of your identity, and gives us the information we need in order to carry out an effective search. LHSA has a maximum of 30 days to respond to the request. Please contact us for more details, or you can access the form from here and send it to us directly.

Please note that LHSA does not hold current or historic general practice records, and we do not hold a hospital record for everyone. 

If you were treated recently, your records may still be kept in a hospital. If this is the case, you can find more information on how to access records still held on NHS Lothian sites here.
