Access to your own records

Under Data Protection legislation, you, the 'data subject', can make a subject access request to view your own patient or personnel records. An application form will be sent out to the data subject or someone acting on their behalf for completion. LHSA has a maximum of 40 days to respond to the request.

Please note that LHSA does not hold GP records. There is also no guarantee that we will be able to locate your hospital records as they may have been destroyed by the hospital at the end of their legal retention period.

As a rule, LHSA collects all pre-1948 health records, but from that date onwards we only take in samples of records.
